Tips for completing the attendance form, accurately & correctly !

 

Notice: Instructors, Notice the important, required information at the top of your attendance form

Include the important information

at the top of your attendance form:

 

  • Date of Class
  • Chapter Number

Also Include Any Important Class Info !

Also Include:

  • If a student is present, please make a check mark, under the class date.
  • If a student is absent, Please mark A
  • If a student makes up a class, Write in the Make up Class Dates, UNDER THE " A "
  • If a student does not make up a class, leave the information blank, until he or she makes up the class…
  • Quiz Scores
  • Mid-Term Test Scores
  • Final Exam Scores
  • Class Percentage Score ( Optional )
  • Classroom FINAL GRADE ( REQUIRED )
