Yellow Cards - Permit Information
Steps to take, in order to complete the "Yellow Cards"
* VERY IMPORTANT NOTE: If a student is absent on the day that your class is completing the yellow cards, make sure that you fill out a yellow card for the student that is absent. Write in the students name and class number on their card. Keep the card with the rest of the students, yellow cards from that particular class. When the student arrives the following class day, give the card to the student and have them complete all of the missing permit information.
How to complete the yellow card
Completing the Yellow Cards: Starting from top of the yellow card to the bottom, have the students fill in all of the information below, correctly, completely & neatly.
Have the students write in the facility control number.
This number is listed on the student's driving permit. It is listed on both sides of the students driving permit. It starts with 3 numbers followed by two letters and
followed by 4 more numbers.
Example 305SC2345 or 278WD5678 or 252EG9012 or 276LB9876
Have the students write in the complete class number. They should write in the actual class number. If any questions, see the database, or contact the driving school office.
Example PLE 000068 or CLA 000079 or MHE 000049
Have the students write in their Social Security Number, under the School Code ( 9772 )
If the students do not have their social security number, tell them that it is not required in order to process the certificate of completion, but they must present a social security card to the drivers license services facility, in order to receive a drivers license. Do not hold up the yellow card, because a student does not have a social security card number.
Have the students write in their area code & phone number, below the social security number.
Have the students write in their Last name on the Yellow card. Make sure they do not write their first name. Tell the students, " Only one letter for each box "
They may add a space if there is a space or an apostrophe in their last name.
Have the students write in their First Name. Tell the students to write their legal first name as it appears on their birth certificate or social security card. If they are called a nickname or a middle name in class, write that nickname & last name on the top of the card, so we can identify the student, easier
Have the students write in their middle initial in the box !
Have the students write in their complete address, under their first name. Make sure that they include all address information. Have them include street, ave, ct. cir, way, blvd., apt #, city / town & zip code. If a student lives at a P. O. Box number, have them include both the P. O. Box Address & the actual address in which they live.
Have the students write in the pemit number on their instructional driving permit. Have the students look on their permit for the permit / license number. It will start with the first letter of their last name, followed by eleven numbers. Have the students write neatly and clearly. If the number is incorrect, the secretary of states office, in Springfield will not be able to issue a certificate of completion.
Have the students write in the Class start date. This is the first date of the class. Look on the attendance form or student roster, and find the class start date. Have the students write in the class start date. Make sure that the date is correct !
Example 05 - 02 - 01 or 12 - 09 - 01
Have the students write in the class finish date. This is the last date of the class. Look on the attendance form or student roster, and find the class finish date. Make sure that the date matches the date on the attendance roster. Call the office if any questions.
Have the students write in their birth date, under " Date of Brith " Have the write in the information in the boxes. Only include month, day & year. Place a zero in the first box of the month if the students were born in January, February, March, April, May, June, July, August, or September. Also, place a zero in the first box of the day that the student was born if the student was born on the first of the month or the second, third, fourth, fifth, sixth, seventh, eight, or ninth. Only have the students write in the last two numbers of the year that they were born. Do not have them write in the year 19 or 20. Do not write in 19 for the year 1999 or 20 for 2000.
Have the students write in male or female. Students need to write in 1 or 2. One for Male, 2 for Female.
Wherever you see status, write in the number 1. Unless the student is classroom only, leave the box, blank. The number one, represents that the student completed drivers education with Northwest Suburban Driving School.
Wherever you see school code number, write in 9772. This is the school code number of Northwest Suburban Driving School.
Note to Instructors: Make sure that you sign all of the drivers education YELLOW CARDS, before you turn the cards into the office. The cards must include your signature.

Yes, make sure that the information is completed for both the classroom and the behind the wheel sections. Complete the information, twice ! Contact the office if any questions.
Have the students write down the number of students in the class. Look on your attendance form & see how many names are listed. Write down the number of the students in the class.
Have the students write in the period length. The period length should be 120 mins.
Students will write in 120 in the period length box.
Have the students write in the number of weeks. If the course is 5 weeks, have the students write in 5. If the course is 4 weeks, have the students write in 4.
Have the students write in the total classroom hours. Students should write in 30.
There are 30 classroom hours. Two hours X fifteen sessions = 30 classroom hours.
Note to the Instructors: On the last day of class, calculate the classroom grades. You will turn in the whole stack of yellow cards at the same time, whether the cards are finished or not We need a classroom grade for all of the students. Before you turn in the yellow cards to the office, make sure that you filled in a letter grade for all of the students yellow cards. classroom grade.
Have the students fill in the date that they received their instructional driving permit.
Tell the students to look on their permit, and write down the issue date on the yellow card. We need this date, in order to file for a certificate of completion.
Have the students fill in the expiration date from their instructional driving permit.
Tell the students to look on their permit, and write down the expiration date on the yellow card.
Have the students write in the number of people that were in the drivers education vehicle, during their driving lesson. Have the students write in 2 3 students.

Have students go down to on-street driving BTW & OBS
Under Period length ( Minutes ), Have students write in 90 minutes for BTW On Street Driving & have students write in 90 minutes for BTW OBS.
Then have the students write in 4 for the amount of lesson for BTW On Street Driving & Have the students write in 4 for BTW OBS.
Next Have the students fill in the number of weeks. Have the students write in 5, if the course was 5 weeks long. Have the students write in 4 if the course was 4 weeks long.
Place a number in both boxes, BTW & OBS.
Finally, have the students write in the total amount of clock hours. In the final two boxes, have the students write in 6 for BTW, and 6 for OBS.
Leave the comments section blank.
Have the students write in the first and last names of one of their parents, or both of their parents.
Have the students write in the phone number where their parents can be reached in case of an emergency. Include the area code.
Note to Instructors: If the student has finished all four driving lessons, write in a final lab grade on their yellow card in the bottom left corner.
Very Important: If a student has not completed all four driving lessons, leave the final lab grade blank. After the student has completed all four lessons, please send a fax to the driving school office ( ASAP ). Indicate the Students Name, final letter grade, and their class number. Contact the driving school office, if you have any questions.
THE BACK OF THE CARDS
Have the student write in their first and last name.
Leave the date, blank. The driving school office will fill in the students driving dates.
Have the students circle the first four, " 2 " This represents Behind the wheel instruction. Under type of instruction, circle 2,in the first box, 2 in the second box, 2 in the third box and 2 in the fourth box. . If you have any questions, leave the box, blank & contact the driving school immediately
Have the students write in 90, four times. This represents the students BTW Instruction time; the students will drive for 90 minutes for each of their 4 driving lessons. In the first box have the students write in 90. In the second box, have the students write in 90. In the third box, have the students write in 90. In the fourth box, have the students write in 90. If you have any questions, leave the box, blank & contact the driving school immediately.
Observation Schedule; Go down to the bottom section of the yellow card
Leave the date, blank. The driving school office will fill in the students driving dates.
Have the students circle the bottom four, " 3 ". This represents Observation.
The students will observe for 90 minutes. Under type of instruction, circle 3,in the first box, 3 in the second box, 3 in the third box and 3 in the fourth box. . If you have any questions, leave the box, blank & contact the driving school immediately.
Have the students write in 90, four times, in the bottom four boxes, at the bottom of the yellow card. This represents the students BTW Observation time; the students will observe for 90 minutes for each of their 4 driving lessons. In the first box have the students write in 90. In the second box, have the students write in 90. In the third box, have the students write in 90. In the fourth box, have the students write in 90. If you have any questions, leave the box, blank & contact the driving school immediately.
The yellow cards are complete & can be turned into the driving school office with the additional items below.
After the course has completed, turn in these items:
Take your time. Make sure that all information is presented neatly, completely & in order.
