Forms for students that are enrolled in the BTW only program
Most students at Northwest Suburban Driving School take the complete course which includes the classroom phase & the Behind the Wheel ( BTW ) lessons. The forms listed below are available for the students that take the BTW ONLY lessons with Northwest Suburban Driving School.
These forms must be completed for students who have taken the classroom phase at their high school & are taking the BTW ONLY portion at Northwest Suburban Driving School. We will also need a photocopy of your driving permit.
